Warning signs missed in WA hospital death
A coroner's report into the 2012 death of a Perth woman has found warning signs of post-surgery complications weren't noticed by medical staff.
Helen Christine MacFarlaine, 52, died from a catastrophic stroke following "uncomplicated and technically successful" elective surgery on an artery in her neck at Sir Charles Gardiner Hospital.
Coroner Sarah Linton said medical staff didn't identify possible developing complications in Ms MacFarlaine, who suffered the stroke the day after she was sent home from hospital.
Ms Linton made no recommendations because individual and organisational changes had already been made within the hospital, including a new clinical deterioration policy to quickly identify and respond to abnormal vital signs.
