* APRIL 2016: SA Pathology chief executive Ken Barr sacked for covering up 100 cases of men receiving false positive results for prostate cancer

* FEBRUARY 2016: More than 20 medical staff revealed to have snooped on patient records - including those related to Cy Walsh, son of slain Adelaide Crows coach Phil Walsh

* NOVEMBER 2015: 10 cancer patients given incorrect chemotherapy doses between July 2014 and January 2015 because of typographical error

* DECEMBER 2014: SA Health admits using cameras hidden in smoke detectors to monitor staff at an Adelaide pathology site suspected of tampering with results

* MAY 2013: A review of more than 50,000 digital breast screens found 72 cancer cases had been missed. Two of these women died.
