Two people with medical conditions have gone missing from an aged care home in the NSW Blue Mountains just a day after each other.

Juliette Broome, 70, left the Leura home on Saturday and is believed to be heading to the inner city suburbs of Ultimo or Glebe.

A day later, 55-year-old Michael Owen was last seen in Leura CBD having attended a church service on Sunday morning.

He has a prosthetic leg and walks with a significant limp, and believed to be heading to Urbenville, almost 900km away in northern NSW.

NSW Police hold grave concerns for the duo as both suffer from an unspecified medical condition.
