More terminally ill Victorians will be able to die in the comfort of their own homes under changes being rolled out across the state.

The Andrews government is putting $7.2 million towards the necessary infrastructure and will trial a new home based model of care which partners hospitals, aged care community palliative care and other health networks.

"All Victorians deserve access to the best possible end-of-life care which relieves pain and suffering and supports families and carers in their last days," said Health Minister Jill Hennessy.
