Woman goes viral after revealing the questionnaire man sent before first date
A woman has revealed the hilarious questionnaire her boyfriend sent to her prior to their first date.
In a video posted to TikTok, Maythe Eleven shared the survey her boyfriend, Graham, asked her to complete before their date. Watch the full clip above.
In the video, Maythe says, "A friend of mine recently asked me out on a date and after I said yes he sent me a Google form."

The impressive form begins by asking details about Maythe's date preferences, starting with what time she would like it to begin, then asking whether she would prefer a dinner date or a day at the beach.
He then jumps into food questions, asking if she prefers steak, burgers or Thai food.
Lastly she is requested to fill out her preferred level of "fancy-schmaniceness" with a scale ranging from one which is "basically pyjamas" to five which is suit and tie.
Upon completion of the form, Graham sent through an official invitation and followed up with an exit interview form once the date had ended.
As the video concluded, Maythe added "Long story short I'm dating someone long distance now."

The video garnered plenty of internet attention, receiving over 2 million likes and a series of mixed reviews.
One person commented, "Oh my God this is terrible," while another wrote, "Dorky and awkward."
However, there were plenty of positive responses with many praising Graham for his effort with one user writing, "This is the kind of men we need, EFFORT."
Another added, "Marry this man."
