POLICE BUGGING SCANDAL: WHAT THE NSW PARLIAMENTARY INQUIRY SAID

* On Deputy Commissioner Nick Kaldas: Report finds a "compelling case" for NSW government to make an apology.

* On Deputy Commissioner Cath Burn: Evidence to inquiry contradicted evidence to earlier investigation but inquiry chair Robert Borsak MP says Ms Burn not the "initial protagonist". "We see her as attempting to do the best and most professional job she could at the time."

* On Commissioner Andrew Scipione: Had "every opportunity" since 2007 appointment to set record straight on Operation Mascot. Mr Borsak said: "He should have provided leadership in this."

* On Ombudsman Bruce Barbour: Combining investigations of legality of Mascot warrants and how the warrants were leaked "has resulted in participants being considered both complainants and perpetrators".

* On police oversight: That a parliamentary standing committee inquire into creating "a single, well-resourced oversight body".

* On surveillance warrants: Government should hold independent review and consider establishing Office of Independent Counsel to test veracity of warrant applications.
