KEY RECOMMENDATIONS FROM GREENOUGH REGIONAL PRISON REVIEW:

* Immediately reassess staffing to minimise lockdowns and ensure the prison can be run safely and securely

* Give prisoners some certainty about their daily schedule and constructive activities when confined to units and cells

* Implement an infrastructure review, prioritising secure storage of tools, ladders and fuel, which were used in the breakout, and medications

* Progress an overhaul of the emergency management plan, with a strong focus on prevention activities

* Improve access to dedicated services and monitor quality of life measures for Aboriginal inmates, who comprise 70 per cent of the prison's population

* Develop strategies for female prisoners including building a separate women's jail on the site that is completely separate from the men's section

* Undertake a capability assessment of emergency response resources, including local groups and the Special Operations Group

* Improve monitoring of prison performance through tools including the "Temperature Report", which gauges inmate mood and its risk to security

* Create a committee that will be accountable for implementing and monitoring any recommendations from internal and external reviews or audits.
