Checklist omission caused WA plane crash

A charter plane that crashed into infrastructure at Perth Airport during take-off last year did not have chocks on the nose wheel and there was no brake pressure when the engine started, the air safety watchdog has found.

The Cobham flight was carrying FIFO workers to the Granny Smith gold mine near Laverton on July 30 when the incident occurred.

The Australian Transport Safety Bureau said crew didn't properly check brake pressure before starting the engine, and the absence of chocks and slight slope of the apron contributed to the aircraft rolling forward.

"This investigation highlights the importance of ensuring all checklist items are addressed," the ATSB said.
