Sargun Ragi, 23, sought police help in 2012 to keep her estranged husband at bay before he finally killed her.

AUGUST

16th - Ms Ragi alleges false imprisonment and rape by Avjit Singh. Police get warrant, bail Singh to appear in court on Aug 20.

Risk assessment report states risk of future violence "likely" and Ms Ragi "vulnerable and in danger".

20th - Interim intervention order made. It prohibits all contact and any attempt to locate Ms Ragi. Ms Ragi starts to contact domestic violence welfare organisations.

24th, 29th and 31st - Singh contacts Ms Ragi

SEPTEMBER

2nd, 8th - Singh contacts Ms Ragi

8th - Ms Ragi reports a breach to police, is told to make statement at a station. Officer calls Singh and leaves a message, telling him to stop calling Ms Ragi and to contact police. No risk assessment report done.

22nd - Ms Ragi reports two anonymous calls, is told to make statement. No risk assessment report done.

25th - Ms Ragi goes to a station to make a statement. Risk assessment form done, states risk of future violence "unlikely".

OCTOBER

4th - Singh goes to his estranged wife's home with a box cutter, knives, rope, fuel and a lighter. Uses wrecking bar to break in, cuts her throat and sets her on fire.

Singh catches alight himself and later dies in hospital.
