Families SA must change: dead tot's gran

The grandmother of Chloe Valentine says nothing has changed within Families SA since social workers missed chances to prevent the four-year-old's death.

Belinda Valentine has called for an independent inquiry into the child protection agency, which was warned repeatedly about Chloe's shocking neglect before her 2012 death.

"How many more need to die before we actually start taking some action?" Ms Valentine asked on Wednesday.

"How many children, how many families need to be damaged and shattered before we act?"

Chloe died of massive head injuries after being forced by her mother Ashlee Polkinghorne to ride a motorbike that repeatedly crashed over a three-day period in the backyard of her Adelaide home.

An inquest into her death found social workers missed repeated opportunities to save her, prompting State Coroner Mark Johns to call for a "massive overhaul" of Families SA.

Ms Valentine said there was no evidence the call had been heeded, adding she feared more children would die unless the agency changed.

She also urged the government to investigate the contact social workers had with a family found murdered near Adelaide.

Families SA have confirmed they had been in "active contact" with Adeline Rigney-Wilson and her children Corey, 5, and Amber, 6, who were found dead in a house at Hillier, north of the city, last week.

Adeline's partner, Steven Graham Peet, has been charged with three counts of murder and will next appear in the Adelaide Magistrates Court in August.

Ms Valentine said an independent review into the agency's contact with the family could be held without prejudicing an ongoing police investigation into the murders.

"It doesn't have to be made public," she told reporters.

"Can we afford to wait two to three years for a coronial investigation into another death? I don't believe we can."

Attorney-General John Rau on Wednesday declined to reveal details of the family's contact with Families SA but said documents had been provided to a royal commission examining SA's child protection system.

Royal Commissioner Margaret Nyland is due to deliver her final report to government in early August.
