MAIN RECOMMENDATIONS

* Mandatory training and quad bike licences/certificates

* Mandatory helmets

* Ban children aged six to 16 riding adult quad bikes

* Ban under-sevens from being passengers

* Develop a star-safety rating system

WHY

* About 200 people died in quad bike accidents in Australia and NZ in the past decade, mostly in rural settings.

* Quad bike accidents are the leading cause of death on Queensland farms.

* Head injuries are the most common cause of death and most victims weren't wearing helmets.

* Helmets are currently optional and licences are not needed for riding on private properties.

THE INQUEST

* Examined deaths of nine people on Queensland properties in the past three years.

* Three were under 12.

* One was a child travelling pillion with an adult relative when the quad bike rolled on an incline. Neither survived.

* Another child was riding alone when her quad bike rolled in a gully and she was crushed.
