After spotting the allegedly stolen Audi in Northcote on Thursday evening, officers followed and eventually tried to intercept it in nearby Preston but the driver failed to stop.
A short time later, the car struck another vehicle and rammed a police car, police say.
No one was injured. Two people were arrested at the scene.
Mark Gilbert, 23, of Watsonia, was charged with 23 offences including theft of a motor vehicle, theft, burglary, driving while disqualified, engaging in reckless conduct, driving in a dangerous manner and assaulting police.
He was remanded in custody to appear in the Heidelberg Magistrates Court on Friday.
A 28-year-old Preston woman was interviewed and released pending further inquiries.
