An elderly man has died after a trauma doctor amputated his legs to free him from under a forklift in the state's north.

The man became trapped under the forklift that had rolled into a creek bed on a property near Tamworth in northern NSW on Tuesday afternoon, police said.

A trauma doctor and paramedics were called to the Moonbi farm, where they amputated the man's legs.
