A targeted shooting in Sydney's west that has left a man fighting for his life in hospital may be linked to drugs, police say.

Three men were sitting in a car in a unit block garage in Guildford on Monday night when a balaclava-clad gunman fired at least two shots through the window.

Ali Arab, 23, who was believed to be in the passenger seat, suffered a gunshot wound to his stomach.

He is in Westmead Hospital in a critical condition.

"This incident does not appear to be linked to any other recent incidents and appears to be a targeted attack," Acting Superintendent Trent King said on Tuesday.

"Obviously it's a callous attack."

It's understood the three men were in a pre-arranged meeting in the garage and had no links to the unit block.

Forensic police were carefully examining the silver Mazda on Tuesday morning.

Two bullet holes could be seen through the passenger side window.

Detectives, including officers from the Middle Eastern Organised Crime Squad, are looking at whether the shooting is drug-related.
