A man is due to be extradited from Queensland and charged with murder after a single-car crash on NSW's north coast in 2015.

The 51-year-old man was driving with a 57-year-old woman on Tweed Valley Way, Tumbulgum, on October 18 when the car left the road, crossed the northern shoulder of the road and crashed into the Tweed River.

The man was able to climb out of the open window before realising his passenger was still inside, police said.

He returned to the car, pulled her out through the window and began CPR until paramedics arrived.

The woman was airlifted to Gold Coast University Hospital and pronounced dead two days later.

Following extensive investigations, NSW Police sought help from Queensland Police in arresting the 51-year-old at a home in Bonogin, Queensland, about 5pm on Monday.

Detectives are expected to go to Queensland and apply for the man's extradition to NSW at a Magistrates Court on Tuesday.
