A car-surfing teenager has suffered broken bones and head injuries after she fell from the bonnet of a moving car allegedly being driven by a drunk driver in Sydney's southwest.

The 17-year-old girl was riding on the front of the sedan in a suburban street in Panania late on Wednesday night when she fell and hit the ground.

A witness who came to help says the teenager was bleeding heavily when she found her in the middle of the road.

"She was on the ground, blood on the head," she told Nine News.

"She was very scared (and) didn't look in a good way".

The girl suffered a broken collarbone and head and hip injuries and was taken to Liverpool Hospital where she remained in a stable condition on Thursday.

The allegedly intoxicated 18-year-old driver was arrested at the scene and charged with low-range drink driving.

His 19-year-old female passenger was uninjured.

NSW Police have ruled out reports the younger girl was struck by the car but say inquiries are continuing.

The man is due to face Bankstown Local Court on August 16.
