Four paramedics responded, and police are investigating.

Officers were told two southbound trucks crashed before two utes travelling behind them collided trying to avoid them.

A third truck towing a trailer jack-knifed afterwards.

The driver of one of the utes, and the driver and passenger of the second ute, were taken to Westmead and Nepean hospitals.

Motorists in the area have been warned to expect significant delays after the motorway closed its southbound lanes.

One of those lanes has now been re-opened.

Queues of up to 10km have been reported, Live Traffic NSW has said.

Diversions are in place, with northbound traffic also heavy.
