Final stage of inquest into Chloe Valentine's death results in heartbreaking testimony

An inquest into the death of four-year-old Chloe Valentine has heard a Families SA supervisor admit "oversights" by his staff.

The organisation had received up to 21 notifications of child abuse claims prior to the toddler's death in 2012 after repeatedly crashing a motorbike she was forced to ride in her Adelaide backyard.

Supervisor Trevor Bailey signed off on a report which said there was no evidence the toddler had suffered abuse at the hands of her mother, Ashley Polkinghorne.

The child was never removed from her mother's care.

Polkinghorne and her former partner Benjamin McPartland are serving jail sentences for manslaughter by criminal neglect.

Today Mr Bailey told the inquest that his staff's investigation into the case was inadequate, but insisted her death could not have been prevented.

"There was no evidence to suggest Chloe was in serious danger," he said.

"Yes, there may have been oversights, but I don't believe the outcome would have been any different."

Mr Bailey said the social workers involved in the investigation did not talk to Chloe or return calls of people who had made child abuse allegations.

The coroner said that the organisation could not have possibly been in a position to determine if Chloe was free of danger if they had not properly investigated the case.

The inquest is in its fifth week and is expected to conclude on February 25.

The CEO of Families SA, Tony Harrison, and the victim's grandmother, Belinda Valentine, are expected to give evidence in the coming days.
