Karara Mining has been fined $40,000 over a 2013 incident that left a worker with a fractured jaw.

Agostino Martino was struck in the face by a scaffolding tube while replacing a broken spreader bar on a screen box at the company's processing plant, 200km east of Geraldton.

Karara Mining pleaded guilty to failing to provide a safe working environment in the Perth Magistrates Court on Thursday.

"The work was being done with inappropriate equipment that put workers at risk, the manufacturer's instructions were not followed, there was no safe work procedure and the job hazard assessment was not compliant with site requirements," Department of Mines and Petroleum state mining engineer Andrew Chaplyn said.
