'UNSATISFACTORY': PASPALEY'S HANDLING OF JARROD HAMPTON EMERGENCY

* The deckhand Alex Evans climbed out onto the boom to pull Mr Hampton up by his air hose, putting his own safety at risk

* The skipper Ronald Watson and engineer Jamie Ray Hulme jumped into the water to retrieve Mr Hampton, leaving no-one driving the vessel, which was still moving

* Mr Watson and Mr Hulme were not wearing any safety gear, including body coverings to prevent deadly irukandji jellyfish stings, putting their own safety at risk

* Mr Watson didn't call 000 and instead phoned OH&S manager Dean Harrison, then communicated with diving medicine specialist Dr Robert Wong

* Paspaley didn't have a man overboard or incapacitated diver retrieval procedure as it didn't consider drowning a high risk because there hadn't been a high rate of it in the industry's recent past
