Police said he packed some belongings and told his family he was heading to Adelaide, in a bid to avoid an upcoming court appearance.

Despite this, his family has not heard from him since that day and there have been no known possible sightings.

He was officially reported missing by 2004.

Sam is described as being 178cm tall with a thin build, short brown hair and green eyes.

Police said detectives have made "numerous appeals for information" in relation to the case since 2004, but he has never been found.
