The five-day search for a missing Victorian mother has ended with a body discovered in a creek.

Fiona Hawker, 47, suffered from a medical condition and did not take her phone with her when she went missing in Monbulk in Melbourne's outer-east on Thursday morning.

A woman's body was found about 3.40pm on Monday.

While the body has not yet been formally identified, it is believed to the missing mother-of-five, police say.

Police are preparing a report for the coroner and the death is not being treated as suspicious.

Police, SES, CFA and Parks Victoria crews scoured the Monbulk town centre and bushland in their efforts to find Ms Hawker.
