Paramedics have performed life-saving CPR on one person who suffered a heart attack following an "horrific" three-vehicle crash on the Cunningham Highway in Queensland's Darling Downs.

The accident, which closed the highway at Allan, near Warwick, has seen two people airlifted to Brisbane in a critical condition and another suffering minor injuries, while two people were uninjured.

The collision, at 2pm on Thursday at the Sandy Creek Road intersection, involved two trucks and a car.

A Queensland Ambulance Service spokesman said a 65-year-old driver was initially rushed to the Warwick Base Hospital with critical head, chest and pelvis injuries.

QAS Toowoomba senior operations supervisor Brad Setch said another person was in cardiac arrest as paramedics arrived at the scene, before being treated and taken to Warwick Base Hospital.

"It was a horrific scene," Mr Setch said.
