Missing police submission could have prevented Man Monis’ bail

A strongly-worded missive from police urging prosecutors to protest Man Monis' successful bail application reportedly never made it to the Director of Public Prosecutions, leaving the Martin Place siege gunman free to roam the streets.

The police submission was passed to a commissioned officer who did not hand it on, News Corp has reported.

Homicide detectives were "furious" Monis had been allowed to walk, a source said.

Monis was on bail on the charge of being an accessory to the murder of his ex-wife.

His partner at the time of his death, Amirah Droudis, was also released on bail charged with the same woman's murder, but had her bail revoked following Monis' deadly actions during the siege at the Lindt cafe in Sydney.

The siege resulted in the shooting deaths of Monis and two of his hostages, Tori Johnson and Katrina Dawson.

An inquest into the siege started yesterday, seeking to establish how the deaths of all three occurred and examine how the siege might have been prevented.

One part of the inquest will be devoted to examining why Monis was out on bail at the time.

State authorities were queried in the aftermath as to how a man facing accessory to murder charges and dozens of sexual assault allegations had been free to walk the streets.

Prime Minister Tony Abbott said it appeared the gunman had "slipped through the cracks".
