An 88-year-old Perth grandmother has died after a tragic accident in her own driveway saw her run down by her husband.

Joan Cullerton was helping her husband Ron pull into their driveway as he was trying to park their car, a routine the couple went through almost every day, but yesterday it went horribly wrong.

Witnesses say that Joan was guiding her husband, who is disabled and in a wheelchair, into the driveway when he accidentally hit the accelerator too hard and hit his wife.

"He was in shock, he kept asking questions: where his wife was, how is she?," a neighbour said.

"It looks like to me that he's mistaken the brake pedal for the accelerator and just gone straight over the top of her."

Neighbours tried to lift the Ford Falcon sedan off Joan but it was too heavy, so they pulled husband Ron out of the vehicle to relieve some of the weight on her.

Fire crews rushed to the scene and worked for more than an hour to free Mrs Cullerton.

She was rushed to Swan Districts Hospital by police escort, but died.

Police are investigating the accident but as it happened on the Cullerton's property a full coronial investigation will also take place.
