A passenger has been fatally thrown from a car which smashed into safety railing and flipped in northern NSW.

The 41-year-old died on his way to hospital after being tossed from the vehicle on Bingleburra Road in East Gresford about 4.40am on Sunday.

Police say the 32-year-old driver was unharmed but taken to John Hunter Hospital for mandatory blood and urine testing.

A woman pedestrian in her 70s also died after an out-of-control car mounted the curb and mowed her down in Sydney's south on Sunday afternoon.

Emergency crews raced to Moate Avenue in Brighton Le Sands just before 1pm after reports a car left the road and hit the woman, who died at the scene.
