Serious fears are held for an 86-year-old missing from Sydney's south-west.

John Ross was last seen leaving an aged care facility on Chiswick Road in Greenacre about 1.30pm on Saturday and still hasn't been found despite multiple police appeals for help.

"Police and family hold serious concerns for his welfare as he lives with a cognitive impairment and other medical conditions," police said in a statement.
