Mason Lee found dead by paramedics at his Caboolture home. The 22-month-old boy is covered head to toe in horrific injuries. Cause of death is inflammation from a ruptured small intestine.

July 10

Queensland government reveals it's investigating the actions of doctors and child protection workers in the lead-up to Mason's death.

July 29

Mason's mother Anne Maree Lee, 27, stepfather William O'Sullivan, 37, and friend Ryan Robert Hodson, 17, are arrested and charged with manslaughter.

August 1

Lee and Hodson are denied bail by magistrates. Hodson later released on August 12 after successful appeal in the Brisbane Supreme Court.

November 23

Three Queensland Child Safety staff are stood down, while another nine face disciplinary action following a department review. Report highlights "serious errors of judgment" were made.
